New Hire Forms
The following paperwork must be complete for new hires:
- W4 Federal Tax Form
- NC4 State Tax Form
- Employment Eligibility Form (I-9) (This form needs to be filled out and returned to HR within 3 days of hire. Employee will have to bring 2 forms of identification with them to the HR office, such as driver's license and social security card, or birth certificate, or a passport .
- North Carolina New Hire Report
- Drug & Alcohol Free Workplace Policy
- Signature Form/Acknowledgment
- Sexual and Other Unlawful Harassment Policy
- Tobacco Free Campus Policy
