Employees who have tested positive for COVID-19, exhibit symptoms of COVID-19, or have been in close contact with a confirmed person diagnosed with COVID-19, or have been in close contact with a person exhibiting symptoms of COVID-19 must complete this Employee COVID-19 Notification Form.
Please check the CDC website for guidance on identifying symptoms.
